Miranda and Jack (Legion and Tali as well) have an argument after you do both of their loyalty missions, I believe. If you don't have enough Paragon (or Renegade, I wouldn't know), then whoever you side with in the argument will remain loyal while the one who you don't pick stops being loyal. If this happens, I think if you gain a higher P/R score, you'll be able to convince them to become loyal again. My problem is that it isn't because it is such a high requirement for it to happen.
I've failed to have a high enough Paragon/Renegade score on both of those events, but I've still never lost loyalty @.@
I wonder if there is other factors at play there.

The problem is that in order to re-earn Miranda's loyalty, you need a damn high Paragon or Renegade score in order to pull it off, and I still don't have it...
BUT I BEAT THE GAME ON INSANITY!!! OH MY GOD, YES!
But three people died in the process because I was rusty on the choices, so I have to try again, this time on Casual. I have to find a way to make Miranda not die, because it doesn't seem possible for me to win back her loyalty. Not this late in the game...

I think if they're not loyal to you, they will die in that final cutscene once the Human Reaper is dead. I've never had it happen to me, but it's worth a try, I guess.

If you don't have Kasumi... and you didn't send Mordin back with the civilians... then I guess I would take him. If you did send him, then I'm not sure on that one.

If you only get one DLC, I would get Arrival.
It prepares you for ME3.

I always send Mordin back to escort the civvies. He has the highest chance of dying in the mission, and his success is almost guaranteed for that bit.
